EmptyStreamingMarkdownFile

Creates a StreamingMarkdownFile whose StreamingMarkdownFile.append checks cancellationToken while parsing, so an append can be aborted. This matters most when the StreamingMarkdownFile.unstableTail is large, since the whole of it is reparsed on every append.
